In iPhone OS 2.x – the current version – Birthday Reminder warns you of upcoming birthdays through email notifications.
You can even configure how many days in advance you want to be notified so you still have time to get a present.
In iPhone OS 3.x – available somewhere this summer – Birthday Reminder can also notify you of upcoming birthdays by the Apple Push Notifications. Yes, the code is already in there, it just needs to be activated as soon as 3.x becomes available.

I agree with you. I entered all of the birthdays and names and it never sends me an email no matter how often I restart it. Anything we can do about it?
